- Contact Information: Phone number and email of a responsible person.
- Consent Number & Validity: Your official MPCB/SPCB Consent to Operate number and its expiration date.

- Current Status: Is the plant currently running, partially running, or shut down?
- Products Manufactured: List key products.
- Major Chemicals Used: Especially those considered hazardous.
3. Hazardous Waste Management: Handling Risks
- Type of Waste: List the categories of hazardous waste generated.
- Quantity: How much is generated (often per day/month).
- Disposal Method: How and where is it disposed (e.g., sent to TSDF - Treatment, Storage, and Disposal Facility).
